Dr. Norma W. Chou and her staff want you to be prepared and know what to expect when you start orthodontic care. On your first visit, you will be asked to complete a confidential questionnaire about your personal information, health history and dental history. You will also be asked to sign a consent form for the collection, use and disclosure of personal information as required by the 2004 government legislation “Personal Information Protection and Electronic Documents Act” (PIPEDA).
During the initial consultation, Dr. Chou will look at your face and teeth, and the associated hard and soft tissues. She will make a general assessment of the orthodontic problem and the treatment possibilities. Drawing on her 35 years of experience, Dr. Chou can determine:
- Type and Severity of Orthodontic Problem
- Types of Appliances Needed
- Timing of Your Treatment
- Length of Your Treatment
- Approximate Cost
If you wish to proceed with a more detailed diagnosis and treatment plan, you will be given another appointment for records, which consists of photographs of the face and teeth, impressions of the teeth, and other measurements. You will also be sent for X-rays of the teeth and jaws. Dr. Chou will then measure the records and discuss the recommended treatment in detail at a third appointment.
